#1.Childbirth. #2.Fisting. #3.Using over-sized sex toys. #4. A Statistical Anomaly 
#1 Women can “bounce back” to nearly normal a few months after childbirth, but some don’t. Usually this isn’t too severe, and can be fixed with a regimen of Kegels. The chances of becoming “too loose” increase after every birth, but aren’t that high unless you have children in a cluster. For example: Four births over a period of four years. 
#2. Fisting, if not done properly, can cause this as well. Fisting should be done slowly with lots of lube, only after the vagina is fully tented, and stimulation during the act should be provided by small, careful movements. The vagina should be given time to recover after each session (at least 3-4 weeks) Extreme fisting (double fisting, punching, etc) should be avoided altogether, if one wants to avoid becoming “loose”. 
#3. Over-sized sex toys will have much the same effect as fisting, but can be even worse (in terms of causing looseness) because of the instinct to thrust while pleasuring oneself with a penis-shaped object. 
#4. Statistical anomaly. These two possible causes are very unlikely, but are possible. Vaginae come in many sizes, so a woman might start out with a vagina that is “too loose”. A woman might also become involved with a man with a statistically improbable penis (in terms of girth), but even so, vaginal intercourse very (very) rarely causes permanent stretching. 
Through the weakening of the muscles that the vagina uses to form around what is inside of it, and through the stretching of the tissue of the vagina itself, a woman can become “too loose”. 
Surgical techniques attempt to fix this by altering the tissue (through techniques designed to repair tears and holes in the vagina), but surgeries should be avoided, as the tissue doesn’t need to be altered in this way to restore tightness. 
By strengthening the muscles in and around the vagina, tightness can be restored. A regimen of Kegels can be used to accomplish this, and they will also help prevent problems that can arise from looseness, such as incontinence and prolapse. 
In some cases, Kegels alone might not be able to produce the desired outcome. If the vagina has been loosened to too great of a degree, the Kegels might only affect the vaginal opening (in terms of tightness), this will leave the rest of the vagina past the opening “loose”. In these cases one should look into devices/regimens made to strengthen the muscles further inside. (These a muscles are what allow the vagina to “form around whatever is inside of it”). Care should be taken when researching/buying/using such devices, and people knowledgeable about them should be consulted. 
-The vagina might be able to pass a baby’s head, but it isn’t magic. People who use birth as an example against the possibility of a vagina stretching, do a disservice to those who have been left loose, and possibly incontinent, by giving birth. (Not every birth results in this, all women are different)
